What is Patellofemoral Pain Syndrome?

The knee joint is one of the largest and most complex joints in the body. It articulates with the patella (kneecap), femur (thigh bone), and tibia (shin bone). The patella is attached to the femur via the quadriceps tendon and to the tibia via the patellar tendon. During knee movement, the patella slides painlessly up and down in the trochlear groove, a surface covered with articular cartilage.

Patellofemoral pain syndrome is a condition that arises when the soft tissues surrounding the patella and the nerves within the bone perceive pain. Commonly referred to as “Runner’s Knee,” it is usually associated with chronic use of the knee joint as a result of running, squatting, or repetitive strenuous activities.

Factors that Cause Patellofemoral Pain Syndrome

The knee joint is subjected to significant stress during daily activities and sports. However, intense and chronic use can lead to several structural issues within the joint:

  • Sudden Changes in Exercise Routine: Abrupt changes in activity frequency, duration, or intensity—as well as switching footwear or training surfaces—can trigger pain.
  • Kneecap (Patellar) Misalignment: If the patella shifts outward from the trochlear groove, pressure on the joint increases, irritating the soft tissues. This can result from hip or ankle misalignments or a naturally high-riding kneecap (patella alta).
  • Muscle Weakness: The quadriceps muscle and tendon are critical for knee movement. Weakness or muscle imbalances can prevent the patella from gliding smoothly, leading to tracking issues.

Symptoms

In addition to the physical structural issues previously mentioned, Patellofemoral Pain Syndrome (PFPS)—often referred to as “Runner’s Knee”—presents with a specific set of clinical symptoms that help Dr. Serkan Sürücü differentiate it from other knee pathologies like meniscus tears or ligament injuries.

Common Symptoms of PFPS

  • Dull and Widespread Pain: Unlike the sharp, localized pain of a meniscus tear, PFPS pain is typically described as a dull ache located behind or around the kneecap.
  • The “Theater Sign”: Pain that worsens significantly when sitting with the knees bent for prolonged periods (such as in a theater or on a long flight). It also intensifies during activities that load the joint, like climbing stairs or squatting.
  • Crepitus: A “cracking,” “popping,” or “grinding” sensation (crepitus) occurring during knee extension or flexion, often caused by the patella not gliding smoothly in the trochlear groove.

Diagnosis

If necessary, X-rays, computed tomography (CT), or magnetic resonance imaging (MRI) may be used to assess the condition of other structures in the joint.

Treatment

Non-Surgical Methods

Most patients can alleviate their symptoms through conservative treatment:

  • RICE Protocol: Rest, Ice, Compression, and Elevation.
  • NSAIDs: Non-steroidal anti-inflammatory drugs used to reduce pain and inflammation.
  • Physical Therapy: Targeted exercises designed to strengthen the quadriceps muscle and improve patellar tracking.
  • Knee Taping and Custom Orthotics: Specialized taping (like McConnell or Kinesio) and shoe inserts to support joint mobility and distribute pressure more evenly.

Surgical Treatment

Surgery is considered for severe pain that does not respond to conservative methods. Under the guidance of Dr. Serkan Sürücü, the following procedures may be performed:

  • Arthroscopic Intervention: A minimally invasive procedure to inspect the knee joint and remove damaged or inflamed tissue (debridement).
  • Lateral Retinacular Release: Loosening the lateral fibers (retinaculum) on the outer side of the knee to correct kneecap instability and reduce lateral tilt.
  • Tibial Tubercle Osteotomy (TTO): Repositioning the bony prominence on the tibia to improve the pull of the patellar tendon, typically used in cases of a shallow trochlear groove or significant malalignment.

Causes and Risk Factors

Patellofemoral Pain Syndrome may not have a single cause. Repeated loading, previous injury, age-related tissue change, muscle imbalance and the demands of work or sport can contribute in different proportions. A risk factor does not establish the diagnosis on its own; the onset, aggravating movements and change over time are considered together.

Symptoms and Effect on Daily Life

Pain, restricted movement, weakness, catching or a sense of insecurity may occur, although not every feature is present in every patient. Pain location, night symptoms, relation to trauma and response to overhead movement or loading help narrow the diagnosis. How Is It Distinguished from Similar Problems?

Tendon, ligament, cartilage, nerve and referred spinal problems can produce symptoms similar to Patellofemoral Pain Syndrome. Examination compares active and passive motion, strength, tenderness, joint stability and neurological findings. Diagnosing the condition from pain location or one provocative test alone can lead to treatment that does not address the actual source.

Clinical Examination and Imaging

Assessment begins with the history and physical examination. X-rays may assess alignment, joint space and bony change; ultrasound can examine tendons and superficial soft tissues; MRI is selected for more detailed soft-tissue or intra-articular assessment. Not every patient needs every test, and imaging findings are interpreted alongside symptoms.

Recovery, Prevention and When to Seek Help

Recovery varies with symptom duration, tissue damage, coexisting health conditions and participation in rehabilitation. Gradual load increases, sound technique and continued strengthening reduce recurrence risk. Deformity after trauma, progressive weakness, fever or redness, new numbness, or severe pain that does not settle needs prompt assessment.
