What is a Tibial Plateau Fracture?

The tibia (shinbone) is the second largest bone in the body and articulates with the distal end of the femur to form the knee joint. The tibial plateau is a flat bony surface located at the proximal end of the tibia, providing a low-friction surface that allows for flexion and straightening of the knee.

A tibial plateau fracture is a fracture of this flat surface and usually results from high-impact injuries (e.g., traffic accidents). The fracture can be a single crack or a splitting of the tibia into several pieces. Individuals with conditions such as osteoporosis or mineral deficiencies are at increased risk of tibial plateau fractures even from lower-impact injuries.

Symptoms

Individuals with a tibial plateau fracture typically experience the following symptoms:

  • Severe Pain: Intense pain located just below the knee, at the upper part of the tibia.
  • Swelling and Bruising: Significant inflammation and discoloration around the site of the fracture.
  • Inability to Bear Weight: Difficulty or total inability to put pressure on the affected leg.
  • Joint Pain: Deep pain within the knee joint itself.
  • Pale Skin: A pale appearance of the skin, which may indicate decreased blood flow to the area.
  • Protruding Bone: In the case of open fractures, bone fragments may pierce through the skin.

Diagnosis

To accurately diagnose a tibial plateau fracture and determine the best course of treatment, Dr. Serkan Sürücü utilizes the following methods:

  • Comprehensive Medical History: Evaluating the mechanism of injury (e.g., a fall, sports collision, or traffic accident) and the patient’s overall health.
  • Physical Examination: Assessing the stability of the knee, checking for swelling, and monitoring the neurovascular status (blood flow and nerve function) of the lower leg.
  • Advanced Imaging: * X-rays: To identify the primary fracture lines.CT Scan (with 3D Reconstruction): Crucial for visualizing the exact shape of the fracture and the degree of bone displacement or fragmentation.MRI: Used to evaluate potential damage to soft tissues, such as the meniscus or ligaments (ACL/MCL).

Through these diagnostic tools, the specific pattern of the fracture, the extent of fragmentation (comminution), and the integrity of the surrounding joint structures are meticulously evaluated.

Treatment

Non-Surgical (Conservative) Methods

Conservative treatment is typically sufficient for minor, non-displaced fractures where the bone remains properly aligned:

  • Immobilization: The knee is stabilized using a cast or a specialized brace.
  • Activity Restriction: Initially, weight-bearing is strictly limited, and movement is restricted to allow the bone to knit.
  • Monitoring: Regular X-rays and follow-up appointments are conducted to monitor the bone alignment during the healing process.
Note: Conservative treatment is also the preferred route for patients with poor overall health or those suffering from chronic skin conditions that may complicate surgery.

Surgical Treatment

Surgery is required when conservative methods are insufficient, if the fracture is displaced, or if it is highly fragmented (comminuted):

  • Internal Fixation: Specialized rods, plates, and screws are used to ensure proper bone alignment and to restore the mechanical function of the knee joint.
  • Bone Grafting: In cases where there is bone loss or structural gaps, an autograft (bone from the patient’s own body) or an allograft (donor bone) may be applied.
  • External Fixation: In certain complex cases, an external fixator (a frame outside the skin) may be used to stabilize the knee before or instead of internal surgery.

Recovery

Following surgery, the healing process is supported through regular follow-up visits and a structured physical therapy program to regain strength and mobility.

Mechanism of Injury and Initial Assessment

Tibia Plateau Fracture can follow anything from a low-energy fall to a high-energy road-traffic or sporting injury. The first assessment checks more than the fracture line: skin integrity, swelling, limb alignment, circulation and nerve function all matter. An open wound, marked deformity, a cold or pale limb, or progressive numbness needs urgent assessment, and the injury should never be manipulated at home.

Symptoms and Associated Injuries

Pain, swelling, bruising and difficulty moving or bearing load are common, but symptom intensity alone does not show whether a fracture is stable. High-energy trauma may also damage ligaments, cartilage, nerves, blood vessels or other bones. The examination therefore considers the whole limb and the injury mechanism rather than focusing only on the most painful point.

What Do X-rays, CT and MRI Show?

X-rays are the first study for defining location, displacement and involvement of the joint surface. CT gives a clearer three-dimensional map of complex or intra-articular fractures and can guide surgical planning. MRI is not routine for every fracture; it is selected when associated ligament, meniscal, tendon or cartilage injury would change management.

When Is Non-Surgical Treatment Appropriate?

A fracture that is acceptably aligned and remains stable may be protected in a sling, brace, cast or splint with scheduled imaging. Safe movement of uninjured neighbouring joints and swelling control are planned during protection. Follow-up images confirm that alignment has been maintained; less pain does not by itself mean the bone is ready for unrestricted loading.

When Is Surgery Considered?

Marked displacement, a step in the joint surface, instability, an open fracture, risk to nerves or vessels, or loss of acceptable alignment may make surgery appropriate. Fixation is chosen for the fracture pattern, bone quality and condition of the soft tissues. The aim is to restore alignment as closely as practical and create enough stability for safe rehabilitation.

Return to work and sport after Tibia Plateau Fracture depends on healing together with motion, strength, balance and the ability to perform the task safely.
