What Is Elbow Osteoarthritis and Stiffness?
Elbow osteoarthritis is a degenerative joint disease in which the cartilage covering the joint surfaces gradually thins and wears away, bony spurs (osteophytes) form at the joint margins, and the joint capsule thickens and stiffens. Because the elbow is not a weight-bearing joint, it is affected less often than the hip or knee; however, when it is involved it causes pain at the ends of straightening (extension) and bending (flexion) and a steadily increasing loss of motion. Pain may be mild in the early stage, but the loss of range of motion becomes noticeable in daily tasks such as eating, combing the hair or reaching into a pocket.
What Causes It and Who Is at Risk?
Elbow osteoarthritis occurs mainly in two forms. Primary osteoarthritis develops without a clear injury, in people who load the joint heavily over a long period and with advancing age; heavy-lifting occupations, those who work with hand tools and people who constantly use arm strength are more prone to it. Secondary osteoarthritis is more common and usually follows a previous trauma, appearing years later.
- Previous elbow fractures and dislocations leave lasting irregularity on the joint surface and set the stage for post-traumatic osteoarthritis.
- Repetitive heavy lifting, the use of impact or power tools and sports that heavily load the upper limb increase the strain on the joint.
- Inflammatory joint diseases such as rheumatoid arthritis and a history of joint infection can accelerate cartilage damage.
- Advancing age and a family predisposition reduce the cartilage's capacity to repair itself and raise the risk.
How Do the Symptoms Appear?
The most common symptom is deep elbow pain that appears at the end points of movement; strain is felt especially when trying to fully straighten or fully bend the arm. Over time, patients notice that they cannot fully extend the arm (loss of extension) or bend it completely. A grinding or friction sensation (crepitus) during movement and swelling in the joint may be seen. In advanced cases, fragments of cartilage and bone that break off and float within the joint (loose bodies) can cause sudden locking or catching episodes. Bony spurs that form on the inner-back part of the elbow can irritate the adjacent nerve and cause numbness and tingling in the ring and little fingers.
What Can It Be Confused With?
Osteoarthritis is not the only cause of elbow pain and stiffness. Pain on the outer side that increases with loading can be confused with tennis elbow, while pain on the inner side can be confused with golfer's elbow; these are tendon problems rather than cartilage problems. The arm giving way or slipping in certain directions suggests elbow ligament injuries, while progressive numbness on the inner side suggests ulnar nerve compression. For this reason the assessment must consider the location of the pain, the pattern of the lost motion and any accompanying findings together.
Diagnosis: Examination and Imaging
Diagnosis begins with a detailed history and physical examination. The doctor measures range of motion by checking how far the arm can straighten and bend, identifies painful points and crepitus, and checks for signs related to the ulnar nerve. X-rays are the first-line imaging that show joint-space narrowing, osteophytes and loose bodies. Computed tomography (CT) is particularly valuable in surgical planning, as it details the position of the bony spurs and loose bodies in three dimensions. MRI can help assess the cartilage, ligaments and surrounding soft tissues.
Non-Surgical Treatment
Treatment of early and moderate elbow osteoarthritis is mostly non-surgical; the aim is to reduce pain, preserve the existing movement and maintain quality of life. Treatment is planned individually for each patient and usually includes the following components.
- Adjusting heavy lifting and repetitive straining movements that increase pain and stiffness (activity modification).
- Short courses of anti-inflammatory medication, on medical advice, to reduce pain and inflammation.
- A personalised physiotherapy programme with gentle stretching that preserves range of motion and strengthens the muscles around the joint.
- Ice and relative rest during flare-ups, with short-term supportive use when needed.
Injections and Biological Treatments
When pain does not settle enough with non-surgical methods, intra-articular injections may come into consideration. Corticosteroid injections can temporarily reduce pain during flare-ups; however, their effect is limited in duration and frequent repetition is not recommended. The role of biological injections such as PRP in elbow osteoarthritis is still being studied and the evidence is limited, so they do not guarantee results. The decision on injections is made individually, according to the stage of the osteoarthritis, the patient's expectations and previous treatments.
When Is Surgery Needed?
Surgery is considered for patients who do not respond to non-surgical treatment and whose pain and loss of motion markedly affect daily life. The choice of method is personalised to the stage of the osteoarthritis, the patient's age and their activity goals.
- Arthroscopic debridement and osteocapsular release: through a keyhole approach the bony spurs are cleaned, impinging loose bodies are removed and the stiffened joint capsule is released to improve range of motion.
- Open surgery: when there are widespread osteophytes, advanced stiffness or marked pressure on the ulnar nerve, an open release and, if needed, freeing of the nerve may be preferred.
- Joint replacement (arthroplasty): reserved only for selected low-demand patients with advanced disease when other methods have been insufficient; it is approached with caution in younger, active patients because of its lifting limitations.
Recovery and Rehabilitation
Recovery varies with the treatment applied and the stage of the osteoarthritis. In non-surgical programmes the aim is to reduce pain and preserve movement within weeks; maintaining the improvement depends on regular exercise. After arthroscopic release, early and regular motion exercises are critical so that the range gained is not lost again; for this reason rehabilitation starts immediately after surgery, under the guidance of the doctor and physiotherapist. The process is guided by the goals reached rather than a fixed calendar, and a forced early return is avoided.
Prevention and Long-Term Outlook
It is not always possible to prevent elbow osteoarthritis entirely, but it is possible to slow its progression. Adjusting movements that place excessive and repetitive load on the joint, working with correct technique, keeping the arm and forearm muscles strong and treating elbow injuries in good time all protect the joint in the long run. When Should You See a Doctor?
Elbow pain and increasing loss of motion that do not settle within a few weeks, that persist at night or at rest, or that affect daily tasks and sleep should be assessed. Progressive numbness or weakness in the hand, sudden locking episodes in the joint or a clear deformity should be examined without delay.
