What is SC Joint Injury?
The sternoclavicular (SC) joint is a critical joint located where the sternum (breastbone) meets the clavicle (collarbone). Due to its exceptionally strong ligaments and joint capsule, injuries to this area are rare, accounting for only about 5% of all shoulder-related injuries.

Classification of SC Joint Injuries

- Grade 1: A mild sprain involving partial stretching or micro-tearing of the sternoclavicular and costoclavicular ligaments. The joint remains stable.
- Grade 2: A subluxation where the collarbone partially shifts out of place, either forward (anterior) or backward (posterior), indicating a significant tear in the ligaments.
- Grade 3: A complete dislocation resulting from the total rupture of the sternoclavicular and costoclavicular ligaments.

- Pain in or around the sternoclavicular joint
- Bruising
- Clavicle instability during activity
- Crackling sounds
- In some cases, difficulty swallowing or breathing, feeling of choking or fullness
How is SC Joint Injury Diagnosed?

Non-Surgical Treatment of SC Joint Injury

- Rest: Avoiding activities that put stress on the shoulder and chest area to allow the ligaments to heal.

- Anti-inflammatory Medications (NSAIDs): Medications such as ibuprofen or naproxen to manage pain and reduce inflammation within the joint capsule.
Surgical Treatment of SC Joint Injury

Causes and Risk Factors
SC Joint Injury may not have a single cause. Repeated loading, previous injury, age-related tissue change, muscle imbalance and the demands of work or sport can contribute in different proportions. A risk factor does not establish the diagnosis on its own; the onset, aggravating movements and change over time are considered together.
Symptoms and Effect on Daily Life
Pain, restricted movement, weakness, catching or a sense of insecurity may occur, although not every feature is present in every patient. Pain location, night symptoms, relation to trauma and response to overhead movement or loading help narrow the diagnosis. How Is It Distinguished from Similar Problems?
Tendon, ligament, cartilage, nerve and referred spinal problems can produce symptoms similar to SC Joint Injury. Examination compares active and passive motion, strength, tenderness, joint stability and neurological findings. Diagnosing the condition from pain location or one provocative test alone can lead to treatment that does not address the actual source.
Clinical Examination and Imaging
Assessment begins with the history and physical examination. X-rays may assess alignment, joint space and bony change; ultrasound can examine tendons and superficial soft tissues; MRI is selected for more detailed soft-tissue or intra-articular assessment. Not every patient needs every test, and imaging findings are interpreted alongside symptoms.
Recovery, Prevention and When to Seek Help
Recovery varies with symptom duration, tissue damage, coexisting health conditions and participation in rehabilitation. Gradual load increases, sound technique and continued strengthening reduce recurrence risk. Deformity after trauma, progressive weakness, fever or redness, new numbness, or severe pain that does not settle needs prompt assessment.
