What is a shoulder fracture?
A shoulder fracture involves bone breaks resulting from trauma to the shoulder joint, sudden force, or a direct impact. These appear as breaks or cracks on an X-ray. Shoulder fractures are classified as non-displaced or displaced:
- Non-displaced fracture: The bone fragments remain in their normal alignment.
- Displaced fracture: The bone fragments are no longer aligned; these cases usually require manual manipulation or surgical intervention.
Which bones are affected by a shoulder fracture?
Fractures in the shoulder joint can occur in three different bones:
- Clavicle (Collarbone)
- Humerus (Upper arm bone)
- Scapula (Shoulder blade)
The most commonly encountered fractures are those of the clavicle and the humerus.
What are the symptoms of a shoulder fracture?
- Severe pain in the injured area
- Tenderness
- Bruising and swelling
- Visible deformity at the fracture site
- In some cases, a “tent”-like deformity under the skin
Shoulder Fracture Diagnosis
If a patient suspects they have a shoulder fracture, immediate admission to an emergency department is recommended. Diagnostic methods include:
- Complete physical examination: Checking for deformity, swelling, and neurovascular status.
- X-ray imaging: The primary tool used to identify the location and type of fracture.
- MRI or CT scans: Used if necessary to examine the fracture’s shape, displacement, and involvement of the joint surface in greater detail.
Shoulder Fracture Treatment
Non-Surgical Treatment:
- Healing Timeline: Most fractures heal without surgery in approximately 4 months.
- Immobilization: The shoulder is stabilized using a sling or similar immobilization devices.
- Rehabilitation: Once pain and swelling are under control, physical therapy is initiated to regain mobility and strength.
When Surgical Treatment is Required:
Dr. Serkan Sürücü makes a surgical decision based on the severity of the fracture, the patient’s age, activity level, and the risk of developing future osteoarthritis. The surgical techniques include:
- Percutaneous Pinning: The use of pins to stabilize the bones in their correct position without a large incision.
- Open Reduction and Internal Fixation (ORIF): Realignment of the bones and stabilization using plates and screws.
- Shoulder Hemiarthroplasty: Replacement of the humeral head (the “ball” of the joint) if the bone is fragmented into too many pieces to be repaired.
Mechanism of Injury and Initial Assessment
Shoulder Fracture can follow anything from a low-energy fall to a high-energy road-traffic or sporting injury. The first assessment checks more than the fracture line: skin integrity, swelling, limb alignment, circulation and nerve function all matter. An open wound, marked deformity, a cold or pale limb, or progressive numbness needs urgent assessment, and the injury should never be manipulated at home.
Symptoms and Associated Injuries
Pain, swelling, bruising and difficulty moving or bearing load are common, but symptom intensity alone does not show whether a fracture is stable. High-energy trauma may also damage ligaments, cartilage, nerves, blood vessels or other bones. The examination therefore considers the whole limb and the injury mechanism rather than focusing only on the most painful point.
What Do X-rays, CT and MRI Show?
X-rays are the first study for defining location, displacement and involvement of the joint surface. CT gives a clearer three-dimensional map of complex or intra-articular fractures and can guide surgical planning. MRI is not routine for every fracture; it is selected when associated ligament, meniscal, tendon or cartilage injury would change management.
When Is Non-Surgical Treatment Appropriate?
A fracture that is acceptably aligned and remains stable may be protected in a sling, brace, cast or splint with scheduled imaging. Safe movement of uninjured neighbouring joints and swelling control are planned during protection. Follow-up images confirm that alignment has been maintained; less pain does not by itself mean the bone is ready for unrestricted loading.
Return to Daily Life and Prevention
Return to work and sport after Shoulder Fracture depends on healing together with motion, strength, balance and the ability to perform the task safely. Reducing fall risk, using appropriate protective equipment and assessing conditions that affect bone health can lower the chance of another injury.
