What Is Tennis Elbow (Lateral Epicondylitis)?

Tennis elbow, known medically as lateral epicondylitis, is an overload injury of the wrist and finger extensor tendons where they attach to the bony bump on the outer side of the elbow (the lateral epicondyle). The origin of the extensor carpi radialis brevis (ECRB) tendon is involved in most cases.

Despite the "-itis" in its name, the condition is now understood as a degenerative tendon disorder (tendinosis) rather than a classic inflammation. Repeated strain causes micro-tears in the tendon, and the repair response produces disorganised collagen fibres and new blood vessels. When that remodelling never completes, the tendon becomes both weaker and painfully sensitive.

Lateral epicondylitis is one of the most common causes of elbow pain and typically affects adults between the ages of 30 and 55. Contrary to what the name suggests, most patients have never played tennis; the problem usually develops in people whose work involves repeated hand and wrist use.

What Causes Tennis Elbow? Risk Factors

Tennis elbow is rarely the result of a single injury. It develops when load accumulates faster than the tendon can adapt to it. The most common contributing factors are:

  • Repeated wrist extension: using a screwdriver, painting, chopping meat, working with scissors or shears.
  • Forceful gripping: holding tools, carrying heavy bags or crates, assembly-line work.
  • Long hours at a keyboard and mouse with the wrist unsupported.
  • Faulty technique in racquet sports, particularly a single-handed backhand played with a loose wrist.
  • Unsuitable equipment: a racquet that is too heavy, the wrong grip size, or strings tensioned too tightly.
  • A sudden increase in training volume or workload over a short period.
  • Weak forearm muscles and poor shoulder or trunk stability.

Smoking, diabetes and older age slow tendon healing and can prolong symptoms or delay the response to treatment.

Symptoms of Tennis Elbow

Symptoms usually begin gradually, and most patients cannot pinpoint when the pain started. Over time the complaints become more defined and begin to limit everyday tasks.

  • Burning or aching pain on the outer elbow, directly over and just below the bony prominence.
  • Pain radiating down the outer side of the forearm towards the wrist.
  • Reduced grip strength: difficulty turning a door handle, opening a jar or shaking hands.
  • Pain when lifting a cup, kettle or tool, sometimes severe enough to drop the object.
  • Marked tenderness when the outer bony prominence is pressed.
  • Brief morning stiffness and a feeling of tightness in the forearm.

Locking of the elbow, visible swelling, pain that persists throughout the night, or numbness and tingling in the hand are not typical of tennis elbow. These findings point towards nerve compression or an intra-articular problem and need separate assessment.

Tennis Elbow vs Golfer's Elbow: What Is the Difference?

Both conditions are overload injuries at a tendon attachment; the difference is which side of the elbow is affected. In tennis elbow the pain sits on the outer (lateral) side and worsens when the wrist is bent upwards. Golfer's elbow (medial epicondylitis) affects the inner (medial) side, and pain increases with wrist flexion and gripping.

Pain on the outer elbow is not always tennis elbow. Radial tunnel syndrome, referred pain from the neck, elbow osteoarthritis and lateral ligament injuries can produce similar complaints. Numbness and tingling on the inner side suggest ulnar nerve compression instead. Clarifying this distinction on examination is what makes treatment effective.

How Is Tennis Elbow Diagnosed?

The diagnosis rests largely on the history and physical examination. The assessment covers how long the pain has been present, which movements aggravate it, occupational and sporting demands, and any treatment already tried.

On examination the lateral epicondyle and the tendon attachment just below it are palpated, and range of motion, grip strength and the neck are also assessed. Three clinical tests support the diagnosis:

  • Cozen's test: with the elbow straight, resisted wrist extension reproduces pain on the outer side.
  • Mill's test: the tendon is stretched by pronating the forearm and flexing the wrist, provoking pain.
  • Maudsley's test: resisted extension of the middle finger loads the ECRB tendon selectively and reproduces the pain.

Imaging is not required in every patient. X-rays may be used to rule out arthritis or calcification within the tendon. Ultrasound shows tendon thinning, thickening and tears. MRI is reserved for persistent symptoms, surgical planning, or when an associated ligament or intra-articular problem is suspected.

Treatment Options for Tennis Elbow

Treatment follows a stepwise plan, and the large majority of patients recover without surgery. The first goal is to settle the pain, the second is to prepare the tendon to take load again.

  • Activity modification: aggravating movements are not abandoned altogether; load and repetitions are reduced so the tendon can recover.
  • Ergonomic changes: increasing tool grip diameter, adjusting keyboard and mouse height, reviewing racquet grip size and string tension.
  • Pain management: short-term analgesics or anti-inflammatory medication, topical gels, and cold application during flare-ups.
  • Bracing: a counterforce forearm strap reduces tension at the tendon attachment, and a wrist splint helps rest the tendon.
  • Physiotherapy: soft-tissue techniques, stretching, graded strengthening and, where indicated, extracorporeal shockwave therapy (ESWT).

Consistency determines whether conservative treatment works. Abandoning the programme after a few weeks is the most common reason symptoms return with the first heavy task.

Exercise and Rehabilitation Programme

Exercise is the single most important part of treatment. The aim is not only to relieve pain but to increase the load-bearing capacity of the tendon so the problem does not return. The programme generally progresses as follows:

  1. Pain control and gentle stretching: extensor stretches performed with the elbow straight and the wrist flexed.
  2. Isometric work: holding a contraction without joint movement, which helps reduce pain.
  3. Eccentric strengthening: lowering the wrist slowly and under control, the key step that stimulates tendon remodelling.
  4. Grip and forearm endurance: soft ball squeezes and forearm rotation exercises.
  5. Shoulder and trunk strengthening: scapular and shoulder-girdle work to reduce the load reaching the arm.
  6. Return to work and sport: technique correction and a graded increase in load back to normal activity.

Mild discomfort during exercise that settles quickly afterwards is expected. Pain that is clearly worse the following day means the load was too high, and the programme should be adjusted.

Injection and Biologic Treatments

If pain persists despite consistent exercise and bracing, injection options may be considered. An injection is not a treatment on its own; it is always planned alongside a continuing exercise programme.

A corticosteroid injection can reduce pain quickly in the first few weeks. However, its long-term results are not superior to conservative treatment, and repeated injections may increase the risk of recurrence, so it is used selectively and sparingly.

Platelet-rich plasma (PRP) injection uses a concentrate prepared from the patient's own blood to support tendon repair and may be considered in resistant cases. Stem cell applications are another option discussed under biologic treatments. Which method is appropriate depends on how long symptoms have lasted, the degree of tendon damage and the patient's goals.

When Is Surgery Needed?

Surgery is relevant for only a small proportion of patients with tennis elbow. It is usually considered when pain persists despite at least six months of consistent conservative treatment and continues to limit daily life and the ability to work.

The aim of surgery is to remove the degenerate tissue at the tendon attachment and allow healthy tendon to reattach to the bone. The procedure can be performed with open, arthroscopic or percutaneous techniques; the choice depends on where the damage lies and whether intra-articular problems are also present.

A short period of protection after surgery is followed by a graded motion and strengthening programme. Return to heavy manual work and racquet sports usually takes several months, varying with the technique used and the demands of the patient's job.

Recovery Time and What to Expect

Tennis elbow is a self-limiting condition in most patients. With appropriate load management and consistent exercise, symptoms usually improve markedly within a few months, while full recovery takes somewhere between six months and a year.

Recovery speed depends on how long symptoms have been present, how severe the pain is, the demands the patient's occupation places on the arm, adherence to the exercise programme, and coexisting factors such as smoking and diabetes. Patients whose symptoms have lasted more than a year tend to recover more slowly.

Less pain does not mean the tendon has fully healed. Recurrence is common when the strengthening programme is stopped as soon as the pain settles.

Prevention and Everyday Precautions

Preventing recurrence is not about removing load from the tendon entirely, but about managing that load deliberately.

  • Lift heavy objects with the palm facing up and the elbow close to the body.
  • Break repetitive tasks into shorter blocks with regular pauses.
  • Increase the grip diameter of tools and racquet handles to suit your hand.
  • Set up your workstation so the forearm is supported and the wrist stays neutral.
  • Keep forearm, shoulder and upper-back strengthening in your weekly routine.
  • Increase training volume or workload gradually, spreading the change over several weeks.
  • In racquet sports, review your technique with a coach, particularly the backhand.

When Should You See a Doctor?

Elbow pain that does not settle within a few weeks despite rest and simple measures should be assessed. Correct load management started early is what keeps the condition from becoming chronic.

  • Pain has lasted two to three weeks and is limiting your daily tasks.
  • Your grip strength has decreased or you are dropping objects.
  • You have numbness or tingling in the hand or fingers.
  • The elbow is visibly swollen or red, locks, or has lost range of motion.
  • Pain wakes you at night.
  • Symptoms keep returning despite previous treatment.
